Permanent Supportive Housing (P H) is an evidence -based, minimal barrier housing
irnteruentiun prioritized fur ind_1viduais with comp l-ex, multi- c►iFri ng conditions that meet
fidelity to established standards. Ind ividuals in P H programs dive with affordability,
autonomy, and dignity through the combination of person -centered flexible, votunta ry
support -services and have a legal right. to remain in their housinig, as defined by the terms of
standard, renewable lease agreement. Access to and maintenance of housing is available to
